Vendor Registration

Becoming a registered vendor with SMWD can streamline the process of doing business with the District. 

Vendors need to fill out an online application that involves submitting various forms and paperwork so that you will be ready to do business with us if such an opportunity becomes available.  Vendors must complete this process prior to doing business with SMWD.

E-mail requests will be viewed between the hours of 7:30am-4:30pm M-F. Any e-mails sent after hours or on the weekend will be replied to during the next business day.

Requirements – Provide the following documents to be attached to your Vendor Assessment Form application:

  1. Copy of your Business License
  2. W9 form
  3. Professional Liability Insurance Certificate (Professional Services only)
  4. Insurance Forms
  5. Public Works Contractor Registration Certification

After we received the vendor’s information, the District will review it and references will be checked.  Along the way, vendors may be contacted for additional information of clarification.  If the application for preapproval is missing any information or the required forms are missing, you will not be preapproved.  Once the vendor file is completed, the vendor will receive a confirmation email.  All applications will be evaluated using standard criteria.  If your application is denied you will be provided with information regarding how to appeal the denial.  If you have any questions, please fill out the contact form to the right and we will be happy to assist you.  

ATTENTION ALL PREAPPROVED VENDORSPlease advise the District of any changes to your company’s address, phone, fax number, Tax ID number, email, web address and other vital information that differs from your original vendor registration forms.  It is your responsibility to update this information.  Failure to do so may result in missed opportunities!
